Part-Time Position (16 hours per week) available for Nurse Practitioners at our On-Site Chronic Disease Management Clinic Located in Grafton, WI.


At Healthstat, we are looking for healthcare professionals interested in joining a company focused on improving health and providing hands-on patient coaching and care. We hire clinicians who want to make a difference. Through the care you provide, you directly impact the health of the employee and the employee’s company. Healthstat hires clinicians who understand the health impact that can be made by managing chronic conditions and providing patients unencumbered access to care.

Clinicians working in Healthstat worksite clinics assist patients in reaching personal health improvement goals. They must be able to quickly assimilate to the employer’s culture, environment and goals. Relationships based on trust, an understanding of the employer’s health benefits program and the local health care community is required. Strong collaborative skills are necessary in order to make the best use of all resources available to employees.

The clinicians must engage employees in personal health improvement, deliver primary care services, and act as a resource and advocate for employee health care needs.

Job Requirements


Education, training, experience:
The clinicians are provided training and support from the Healthstat clinic operations team. Clinic operations are achieved through the use of a state of the art electronic medical record program and predictive modeling tools.

o Nursing Skills (Technical and Functional)
o Problem Solving/ Analytical Skills

o Must by proficient in Phlebotomy
o Must be licensed as a Nurse Practitioner in the state of Wisconsin.
o Must have Two - Five years of professional clinical experience.


o Knowledge and experience in health risk assessment, lifestyle risk reduction, disease management, care management and health promotion is a plus.
o Experience in Urgent Care and Occupational Medicine a plus.
o Previous work experiences in an independent environment a plus.
